The State of Louisiana along with the CDC now recommends that people whose immune systems are compromised moderately to severely and are fully vaccinated with an mRNA (Pfizer or Moderna) COVID-19 vaccine should receive an additional dose of that same mRNA COVID-19 vaccine.

If you meet the criteria, please print and complete this form and bring it with you to your vaccine appointment along with your vaccination card. Copies of this form will also be available at all of our vaccine sites.

No documentation from a healthcare provider is needed to receive the third dose, though we recommend you speak with your provider to see if getting an additional dose is appropriate for you.
